Breast implants are prosthetic devices placed into the breast pocket, causing the breasts to increase in size immediately. While many women in Boston opt for smaller size implants these days, the procedure remains the most popular elective surgery in the world. Great, natural-looking results can be achieved by factoring in a woman’s body and individual anatomy when selecting the right breast implant.
For the right candidates, breast fat-grafting is an excellent all natural alternative to breast implants. Dr. Lee uses liposuction to extract fat from areas such as the abdomen and flanks. He then injects the fat into the breasts. Fat grafting is an excellent option for improving fullness in the upper half of the breast. Depending on the amount of fat that can be harvested, the results can be quite substantial, achieving an increase of up to one bra cup size. Some women who are very thin may not have enough fat to inject into the breast. Fat has an advantage of looking and feeling extremely natural and realistic. Not all the fat that is injected will “take” - meaning some of the fat will be absorbed by the body. However, once the fat graft takes, it becomes part of the natural breast tissue. Although some women may require two sessions of fat grafting, many patients are very happy with one treatment.
The primary goal of breast augmentation is to enhance the size of breasts that are too small for the woman’s liking. Breasts lifts, on the other hand, lift and reshape sagging breasts so they appear perkier without altering the size. If you have drooping breasts and desire more fullness, Dr. Lee may perform a breast lift and augmentation together.
When it comes to your Boston breast augmentation, your consultation is the most crucial step of the process. That is when you will decide whether you and your surgeon’s team are aligned. During your appointment, Dr. Lee and his team will ask about your cosmetic goals, take pictures and measurements, and walk you through the available options. He will also answer your questions and let you know if you are eligible for the surgery.
After learning about your goals, he will create a surgical plan designed specifically for you. His clinical recommendations are based on your chest measurements, existing breast tissue, and whether you have other issues, such as two different breast sizes or breast droopiness. Preparing for our Boston breast augmentation is fairly simple. You will need to stop taking NSAIDs, Vitamin E, and medications that can increase bleeding. Mammograms are usually only necessary if you have a family history of breast cancer or are older than 40 years old. If you smoke, you’ll need to quit at least 4 weeks before surgery and stay smoke-free at least 4 weeks after surgery. Take precautions to prevent pregnancy leading up to your operation, as you won’t be able to have surgery if you are pregnant.
Measured in cubic centimeters, breast implants come in a variety of sizes. The best implant size for you comes down to how much natural breast tissue you have now, your breast width, your chest anatomy, and how big you want to go. You are welcome to bring inspirational photos to give Dr. Lee a sense of your preferences. You will also have an opportunity to try on various size implants in the office using our implant sizers so that you can get an idea of what your augmented breasts would look like in the mirror.
Both silicone and saline implants have outer shells made of silicone. Silicone implants are filled with a silicone gel that is quite cohesive - similar to a gummy bear. Saline implants are filled with sterilized saltwater. Both are biocompatible, so if they do rupture or leak, they won’t pose a serious threat to your health. Saline implants usually cost less than silicone implants; however, they may not feel as soft.
Round breast implants provide more cleavage and a rounder shape. Teardrop implants have a soft slope at the top, similar to natural breasts. Teardrop shaped implants have fallen out of favor recently due to the issues with texturing and implant rotation. Most women choose to go with round implants.
Smooth implants feature a smooth shell, allowing them unobstructed movement in the breast pocket similar to natural breast tissue. Textured implants have a rough surface that helps them grip better and stay in place. Teardrop implants always have a textured shell. Round implants usually have a smooth surface. Textured implants are less commonly used but can be an option for complex revision cases. Most women who get breast augmentation go with smooth implants.
High-profile implants are best for patients desiring maximum forward projection and those with narrow chest walls. Moderate-profile breast implants offer a tasteful amount of outward projection and are suitable for the majority of women. Low-profile implants may be necessary for women who have wider chests, for patients who lack upper pole fullness, or those hoping for a more subtle and less projecting result.
Submuscular implants are placed under the pectoralis muscle, providing a layer of coverage to make the implant edge at the top of the breast less obvious. Subglandular implants go above the muscle and are a good option for patients with enough breast tissue to hide the implant. Dr. Lee may suggest the dual-plane approach, which means only the upper part of the implant goes below the muscle. Subfascial implant placement, a newer technique, may also be an option. With this method, he places the implants under the fascia, a thin layer covering the muscles.
It is normal to experience a mix of emotions on the day of your surgery, from excitement to nervousness. Before heading into the operating room, Dr. Lee will meet with you to ensure you feel good about your decisions. You can rest assured he will take the time to answer any questions or concerns so you feel relaxed as you head into your operation.
Breast implant surgery takes place under general anesthesia. The operation typically lasts one to two hours. Dr. Lee will make incisions in accordance with your surgical plan, dissect a pocket for the implants, and place them into the ideal position. After checking your implants for symmetry and alignment, he will close the incisions with fine sutures and surgical tape.
Choosing an incision is ultimately between you and your surgeon. The size and style of breast implant matter as much as your preferred scar location. Dr. Lee will explain your options in detail, ask for your input, and together, you will reach a decision on the breast implant incision type for your specific needs.
Dr. Lee will mark out the areas for liposuction as you stand upright. Once you are under general anesthesia, he will inject tumescent fluid to decrease bleeding and isolate the fat cells. Next,he will break up the fat with a liposuction cannula, then extract it through a suction tube. Before injecting the fat into your breasts, he separates the fat cells from the surrounding fluid so that the fat is as pure as possible. The operation usually takes one to three hours, depending on the extent of your liposuction.

Fat transfer patients won’t have to worry about replacing breast implants, although the breasts may change over time due to aging, pregnancy, changes in skin elasticity, or other factors. Maintaining a stable weight is the best way to keep the fat cells from shrinking or enlarging.
Most women report that breast augmentation does not interfere with breastfeeding. Some have found breastfeeding to be challenging.
Patients go home the same day after breast augmentation surgery. An overnight stay is rarely necessary but can be an option if you undergo multiple procedures at once or have a heightened risk of complications due to pre-existing or emergent medical issues.
Most women find that breast implants do not interfere with milk production. However, a small number of patients find breastfeeding a bit more challenging after receiving implants. Discuss whether you plan to breastfeed with Dr. Lee during your consultation. He may suggest a specific implant style, incision technique, or implant placement with this in mind.
Changes in nipple sensation can occur, but most patients do not experience lasting issues.
